Athletic Training Forms
Student-athletes are required to complete the online health history form and submit two forms below in person in order to be eligible to compete athletically for Salisbury University. The physical examination form and the consent of disclosure form are below. Please complete the consent form and have the physical exampination form printed and ready during the time of your physical and screening. As a reminder, if you are obtaining a physical from your own physician, you must use the provided physical examination form and the physical cannot be obtained 30 days or more prior to your first practice. Physicals are only required of freshman and transfer student-athletes.
